Unlike in the original Boxhead game where the player has to rescue civilians in a city, in Boxhead: The Rooms, the player is instead to just to trying survive as long as possible in an arena of their choice.

The player’s main objective in this game is to attain as high of a score as possible. Player’s can earn points by killing zombies, with each zombie kill being worth 100 points. However, if they can kill enough zombies in a short period of time, they can earn a multiplier that will allow them to accumulate points faster.

The player starts each arena with a pistol that has infinite ammo and can go on to unlock better weapons by raising their score multiplier to different levels. There is a much larger variety of weapons in The Rooms compared to the first game in the series, as the player can unlock a total of 8 weapons compared to the 4 available in the original game.
